Signup Dates:
All sign-ups will be held at Rays Superfoods in North Judson and will be on the following
dates and times:
Wednesday July 21 6 pm to 8 pm
Thursday July 22 6 pm to 8 pm
Friday July 23 6 pm to 8 pm
Saturday July 24 1pm to 4pm
Monday July 26 6 pm to 8 pm
Tuesday July 27 6 pm to 8 pm
Wednesday July 28 6 pm to 8 pm
July 28 is the final day for signing up.
Please bring with you a copy of your childs birth certificate and latest report card.
We will try to have a Dr. available on a certain date for physicals. Physicals will be the responsibility of the parents.
